Suffolk County Sheriff’s Office: Medical ID Card Signup

The Suffolk County Sheriff’s Office assists seniors or those with medical needs to keep all required medical information available on a small card that you can carry in case of emergency. Please bring a list of your medications, emergency contact information, doctor information, and a list of allergies you may have.
